Catch-Up Patients on Immunizations and Other Important Screenings with the Help of Ohio AAP & Ohio’s MCOs!
We know the last year and half has presented some unprecedented challenges for pediatric healthcare. Now, more than ever, it is of the utmost importance to make sure that your patients are up to date on well care and immunizations.
The Ohio AAP in partnership with Ohio’s Medicaid Managed Care Organizations (MCOs) have been working to support primary care practices in piloting innovative interventions in practice by helping to improve access to well care and immunizations for your pediatric population.
We would like to partner with practices to provide EXCLUSIVE resources, materials, and access to support over the next several months to help engage your patient population in well care and “catch-up” on immunizations and other important screening and prevention efforts through well care in the primary care medical home.
Join us in this important project and receive:
- Exclusive access to FREE resources for providers and patients and families on lead screening, dental care, nutrition and exercise, depression screening and more. These resources will assist providers in discussing important well care, prevention and immunization topics and help patients access important resources, education, and support
- Exclusive assistance with setting your practice up to receive the covid-19 vaccine
- Access to FREE outreach materials, including postcards, and social media messaging. Ohio AAP will cover printing and mailing costs.
- FREE virtual or in-person training on evidence-based interventions to increase immunization rates and supporting resources
- Access to practice coaching and data support
- Training on developing and implementing clinic strategies for all clinic staff to focus on reminder and recall strategies and scheduling priorities
- Educational materials for families on the importance of WCC and sports physicals and posters and educational materials for the clinic
